Enrollment Disclaimer

Important things you should know

Here is some important information you should know before you enroll in one of our Prescription Drug Plans. Please make sure to read this section carefully.

  • To be eligible, you must be eligible for Medicare Part A or enrolled in Medicare Part B. You must also live in the plan’s service area.
  • Network pharmacies include retail, mail-order, specialty, long-term care, home infusion and Indian Health Services pharmacies. To obtain names and addresses of WellCare’s network pharmacies, use the pharmacy locator tool on our Web site at www.wellcarepdp.com or current members contact Customer Service at 1-888-547-5252, prospective members call 1-888-908-5252 (TTY users call 1-888-816-5252), Monday – Sunday, 7am to 2am Eastern, P.O. Box 69328, Harrisburg, PA 17106-9328.
  • You must use contracted network pharmacies to access your prescription drug benefit, except under non-routine circumstances (e.g., emergencies) when you cannot reasonably use network pharmacies.
  • For more information on mail-order prescription drug service, current members contact Customer Service at 1-888-547-5252, prospective members call 1-888-908-5252 (TTY users should call 1-888-816-5252), Monday–Sunday, 7am to 2am Eastern. For drugs on our formulary, you can use the plan’s network mail-order services. Our plan’s mail-order service requires you to order at least an 84-day supply of the drug and no more than a 93-day supply.
  • People with limited incomes may qualify for extra help to pay for their prescription drug costs. If you are eligible, Medicare could pay for seventy-five (75) percent of drug costs including monthly prescription drug premiums, annual deductibles and coinsurance. Additionally, those who qualify will not be subject to the coverage gap or a late enrollment penalty. Many people are eligible for these savings and don’t even know it. For more information about this extra help, contact your local Social Security office or call 1-800-MEDICARE (1-800-633-4227), 24 hours per day, 7 days per week. TTY/TDD users should call 1-877-486-2048.

  • If you choose to have your plan premium deducted from your Social Security check or if your plan premium is currently deducted from your Social Security check and you would rather receive a direct bill, it could take up to three months to take effect. You are responsible for ensuring full payment of your plan premium.
